  7. Kaarya/vivad sambandhi vivaran — jahan form mein kaam/arz/maang likhni ho, wahan spasht aur sankshipt roop se likhen; dates aur jagah ka zikr karein.
  8. Anya lagoo document/saakshya — agar documents attach karne ko kaha ho (ID proof, address proof, receipts), unki list aur copies jodhen.
  9. Signature aur designation — form ke ant mein apna signature, naam (print), aur agar applicable ho to designation/office stamp lagayen.
  10. Fee aur payment proof — agar form fee lagti ho to receipt ya challan ki copy jod kar payment details likhen.
